When not to take HEXALGIN® solution: You should not take HEXALGIN® solution if you suffer from any of the following conditions, as this medicine can cause serious problems in these cases:
- known hypersensitivity (allergy) to the active ingredient metamizole, other pyrazolones (eg phenazone, propifenazole) or pyrazolidines (eg phenylbutazone, oxyfenbutazone), as well as to any of the other ingredients
- patients with known intolerance to analgesics (analgesic asthma or analgesic intolerance such as urticaria-angioedema). Those patients who respond to analgesics (analgesics) such as salicylates, paracetamol, diclofenac, ibuprofen, indomethacin or naproxen with bronchospasm (bronchospasm resulting in difficulty breathing) or other hypersensitivity reactions.
- acute, recurrent hepatic porphyria (a rare metabolic disease) as there is a risk of an attack of porphyria
- congenital glucose-6-phosphate dehydrogenase deficiency (a rare metabolic disease) due to a risk of haemolysis (destruction of red blood cells)
- impaired bone marrow function (eg after treatment with cytostatics - products or diseases of the haematopoietic system (impaired haematopoiesis)
What special precautions should be taken: HEXALGIN® solution contains the pyrazolone derivative metamizole and includes a rare but life-threatening risk of shock (circulatory collapse) and agranulocytosis (a severe disease associated with a significantly reduced number of some white blood cells).
When you can take HEXALGIN® solution only after consulting a doctor: For children under 12 years of age, the product should only be used with a doctor's prescription. The risk of possible severe hypersensitivity reactions to HEXALGIN® solution is significantly increased in the following groups of patients:
- patients with bronchial asthma, especially if they suffer from rhinosinusitis (inflammation of the nose and nasal sinuses) and nasal polyps
- patients with chronic urticaria
- patients with hypersensitivity to colorants (eg tartazine) or preservatives (benzoates)
- patients with alcohol intolerance. Such patients respond to even small amounts of alcohol with symptoms such as sneezing, tearing and severe flushing. Such alcohol intolerance may be evidence of undiagnosed analgesic asthma syndrome.
Before using HEXALGIN® solution, it is necessary to inform the treating physician if you suffer from any of the listed diseases or conditions. In patients at increased risk of hypersensitivity reactions, HEXALGIN® solution should only be used after careful consideration of the benefit / risk balance. If HEXALGIN® solution is used in these cases, the treating physician should monitor the patient closely and be prepared to provide emergency care.
HEXALGIN® solution may cause a decrease in blood pressure The risk of such reactions is also increased in: patients with low blood pressure (hypotension) or fluid loss, circulatory disorders or initial circulatory collapse, as in patients with myocardial infarction or severe injuries with high temperature
In patients in whom it is mandatory to avoid lowering blood pressure, such as patients with severe coronary heart disease or narrowing of the cerebral vessels leading to impaired irrigation, HEXALGIN solution should only be used with careful monitoring of cardiovascular function.
Metamizole should be used with caution in patients with renal or hepatic impairment.
What other products affect the effect of HEXALGIN® solution or may be affected by the product: HEXALGIN® solution may decrease the plasma concentrations of cyclosporine (a product that suppresses immune responses in organ transplants), therefore cyclosporine levels should be monitored in simultaneous application with the product.

What foods and drinks should you avoid:

Co-administration with alcohol may lead to mutual potentiation of the action of the two products.

What children and adults should consider when using HEXALGIN® solution: The product should not be used in infants less than 3 months of age or less than 5 kg body weight, as there is insufficient clinical experience.
Elderly patients should be considered for possible renal and hepatic impairment.
What special precautions should be taken during pregnancy and lactation: The use of HEXALGIN® solution during the first 3 months of pregnancy is not recommended. HEXALGIN® solution should only be used for the next 3 months after careful benefit / risk assessment. HEXALGIN® solution should not be used during the last 3 months of pregnancy.
The product suppresses the natural function of platelets (platelet aggregation), which can lead to increased bleeding, especially at birth. Premature closure of an important blood vessel for the unborn child is possible (the so-called Botal duct, which closes naturally only after birth).
Breast-feeding is not allowed during the first 48 hours after taking HEXALGIN® solution, as the breakdown products pass into breast milk.
What to consider when driving, operating machinery or in an unsafe position (eg climbing stairs): There are no known disturbances in concentration and reactivity at the recommended dose. As a precautionary measure, especially at high doses, driving, operating machinery or other hazardous activities should be avoided, especially after alcohol use. HEXALGIN® contains the pyrazolone derivative metamizole, where, although rare, there may be a risk of life-threatening shock. and agranulocytosis.
Posology, method and duration of administration: Unless otherwise prescribed by your doctor, the following dosages are recommended. Please follow the instructions for use, otherwise the HEXALGIN® solution will not work properly.
At what dose and how often you can take HEXALGIN® solution: The following dosing schedule is based on a ratio of 1 ml of solution corresponding to 20 drops (when the vial is held upright). The prescribed single doses can be administered up to 4 times a day.
body weight / age single dose
adults and adolescents over 15 years 20 - 40 drops

children 9 - 15 kg (1 - 3 years) 3-10 drops

Renal or hepatic impairment: High doses should be avoided as the rate of excretion is reduced in renal or hepatic impairment. No dose adjustment is required for short-term use. There are insufficient data on long-term dosing in patients with impaired renal or hepatic function.
Renal and hepatic impairment should be considered in the elderly and in patients with deteriorating general condition.
When and how you can take HEXALGIN® solution: HEXALGIN® solution for oral administration should be taken with a sufficient amount of liquid.
How long you can use HEXALGIN® solution: It is recommended that you do not take the product for more than 3 days as an antipyretic (for fever) and for 5 days as an analgesic (painkiller) without consulting your doctor or dentist.

What to do if HEXALGIN solution has been taken in very large amounts (intentional or unintentional overdose): If too much HEXALGIN® solution is taken, nausea, abdominal pain, impaired kidney function and in rare cases may occur. cases dizziness, drowsiness, unconsciousness, seizures, low blood pressure to shock and arrhythmia.
Please tell your doctor if you have taken a large amount of HEXALGIN® solution at one time, and he / she will give you advice and take the necessary measures if necessary.
Note: After taking very high doses, the excretion of a harmless metabolic product (rubazonic acid) may lead to red discolouration of the urine.
What to do if you take less than the prescribed dose of HEXALGIN® solution or you miss a dose: If you forget to take a product, you should not take more than the usual dose for the next dose.
